Hazel was born July 21, 1928 to the late John and Melvin Conley in Salyersville, KY. Besides her parents, Hazel was also preceded in death by her husbands: Rev. Woodrow Hackworth, whom Hazel married on November 26, 1943 and Virgil Woodbury; daughter: Rissie Hackworth; sons: JR Hackworth and Rev. John Hackworth; daughter-in-law: Phyllis Hackworth; grandson: John Roger Hackworth Jr.
